Primary Care Physician – Virtual Primary Care Practice

Job Summary
Conduct comprehensive, relationship-based primary care through telehealth, including preventive, acute, chronic, and complex care for adult patients aged 18+. Manage medication regimens, perform diagnostic evaluations, and coordinate transitions-of-care with specialists while collaborating on telephonic calls. Deliver longitudinal care within an interdisciplinary model supported by integrated behavioral health, social services, and population health resources using the Epic electronic health record. Maintain Pennsylvania and New Jersey medical licenses and commit to value-based care initiatives within Jefferson Health's virtual practice.
Required Qualifications
- MD, DO, or equivalent medical degree
- Board certification or eligibility in Family Medicine or Internal Medicine
- Pennsylvania and New Jersey medical licenses, or the ability to obtain both by the start date
- Commitment to delivering an outstanding virtual patient experience
- Commitment to health equity and caring for medically and socially diverse populations
- Strong communication and collaborative-care skills
- Two or more years of post-residency clinical experience
