Job Summary
Conduct thorough in-person primary care assessments for a manageable caseload of 8–10 patients daily, focusing on deep clinical evaluation and relationship building. Lead interdisciplinary team meetings to craft and execute personalized, holistic care plans that prioritize patient independence and home-based health. Oversee transitions of care, including acute episodes and long-term placement management, while acting as the primary advocate for patients within Medicare and Medi-Cal systems. Maintain a standard 8:00 AM – 5:00 PM schedule with a limited phone-only on-call rotation. This role offers a clear trajectory toward Medical Director positions for clinicians passionate about value-based medicine.
Required Qualifications
- Completion of an accredited residency in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
- Active, unrestricted state physician license
- CDS certification
- DEA registration
- Ability to maintain CPR and First Aid certification
- Eligibility to serve Medicare and Medi-Cal participants
