Job Summary
Conduct primary care services for a panel of approximately 1,200 patients within a Patient Aligned Care Team Model, managing chronic disease and preventive health for non-surgical adult patients. Evaluate, diagnose, treat, and provide consultation for a wide variety of diseases, injuries, and disorders, while assessing and stabilizing patients with emergent conditions consistent with medical staff policy. Collaborate with mid-level providers and other specialties to manage or co-manage patient health, utilizing on-site mental health services or consulting external subspecialties as needed. Work Monday through Friday from 8am to 4:30pm, excluding federal holidays, with no weekend call requirements.
Required Qualifications
- U.S. Citizenship
- Unrestricted U.S. Medical License
- Successful completion of an Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA) accredited residency in Internal or Family Medicine
