Primary Care Physician [Spanish Required]

Provide equitable value-based primary care to local Medicare populations at Oak Street Health’s neighborhood clinics. Assess and diagnose patients, oversee and direct primary care, prescribe treatments, and maintain patient EMR data via Canopy and Greenway. Collaborate with regional/central leadership to meet health quality goals and work with Practice Managers to direct the center care team. New graduates are encouraged to apply. Role may include potential Center Medical Director consideration; position emphasizes outpatient, value-based care for older adults with a focus on quality and patient experience. 40 hours per week, full-time; bilingual Spanish required.
