Primary Care Physician [Spanish Required]

Provide equitable, value-based primary care to local Medicare patient populations within Oak Street Health’s neighborhood primary care centers. Assess and diagnose patients, oversee and administer primary care, prescribe medications, maintain Electronic Medical Record data via canopy and greenway, and collaborate with regional/central leadership to meet health quality goals. Work with Practice Managers to direct and manage the center care team. New graduates are encouraged to apply; there is also potential consideration for a Center Medical Director role. Spanish bilingual proficiency is preferred. The role emphasizes delivering preventive care, improving patient experience, and functioning within a value-based care model.
