Job Summary
Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions for an established panel of predominantly geriatric patients. Deliver preventive care, wellness visits, and health screenings with a focus on early intervention. Develop and manage individualized care plans while collaborating closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care. Utilize electronic health records to document clinical encounters and track patient outcomes. Engage in quality improvement initiatives and educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices. Monday–Friday, 8:00 AM – 5:00 PM.
Required Qualifications
- MD or DO from an accredited medical school
- Internal Medicine or Family Medicine or Geriatric Medicine
- Active, FL license
- Current DEA registration
- At least 3 years of experience in Outpatient Primary Care
- Strong clinical judgment with a focus on preventive and longitudinal care
- Excellent interpersonal and communication skills
- Commitment to a team-based, patient-first care model
