Job Summary
Perform comprehensive in-person assessments and partner with the interdisciplinary team to develop personalized care plans for vulnerable seniors. Coordinate discharge for acute, short-term, and long-term placement while focusing on holistic care without hospital rounds, seeing an average of eight patients daily. Maintain bilingual proficiency in Spanish and English, hold an active state physician license, and complete required CPR and First Aid certifications. Regular hours include 8am to 5pm Monday through Friday, with rotating on-call availability for a few nights or weekends per month. This role offers growth opportunities to become a medical director within a mission-driven team dedicated to unlocking the full potential of seniors through community-based care.
Required Qualifications
- Bilingual in Spanish & English
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Master's or doctorate Degree
- Active state physician license without restrictions
- State CDS certification
- DEA registration
- CPR and First Aid certification
- Eligible for enrollment to provide services to Medicare and Medi-Cal participants
- COVID-19 vaccination or vaccine declination form
