Job Summary
Perform comprehensive in-person assessments and partner with the interdisciplinary team to develop personalized care plans for vulnerable seniors. Coordinate discharge for acute, short-term, and long-term placement while focusing on holistic care with an average of eight patients daily. No hospital rounds are required, allowing for dedicated home visits and community-based support. Regular hours include 8am to 5pm, Monday through Friday, with a rotating on-call schedule involving a few nights or weekends per month. This role offers growth opportunities to become a medical director within a mission-driven program that prioritizes both patient impact and provider well-being.
Required Qualifications
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Master's or doctorate Degree
- Active state physician license without restrictions
- State CDS certification and DEA registration
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
- CPR and First Aid certification upon hire preferred
- Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants
- COVID-19 Vaccination Policy: proof of vaccination, or a vaccine declination form will be required prior to employment
Desired Qualifications
- Board Certification preferred
- Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
