Primary Care Physician – Pittsburgh

Provide outpatient primary care services across Pittsburgh neighborhoods including Eastern Neighborhoods, Westmoreland County, Downtown, and Squirrel Hill. Deliver medical management for family physicians, geriatricians, and general internists within a value-based care model emphasizing preventative care and long-term wellness. Coordinate with extended care teams comprising mid-level support, behavioral health consultants, social workers, and dietitians. Manage correctional medicine duties at Allegheny County Jail as required. Maintain board eligibility or certification and hold valid Pennsylvania licensure, DEA, and CPR certifications prior to employment.
