Job Summary
Provide outpatient primary care services across diverse Pittsburgh neighborhoods including Eastern Neighborhoods, Westmoreland County, Downtown, and Squirrel Hill. Manage patient care within a value-based model that emphasizes quality outcomes, preventative care, and long-term wellness. Coordinate with extended care teams comprising mid-level support, behavioral health consultants, social workers, registered dietitians, and certified diabetes educators. Deliver correctional medicine services at the Allegheny County Jail. Maintain board eligibility or certification, hold a Pennsylvania license, and possess DEA and CPR certifications.
