Primary Care Physician at Oak Street Health provides equitable, value-based care to Medicare populations at a neighborhood primary care center. Responsibilities include assessing and diagnosing patients, overseeing and directing primary care, prescribing treatments, maintaining EMR data, collaborating with regional leadership to meet health quality goals, and directing the center care team in a collaborative outpatient setting. Must hold MD or DO, board certification (or eligibility) in Internal or Family Medicine, active state license, active DEA license, and US work authorization.
