Primary Care Physician-OSH

Primary Care Physician at Oak Street Health provides equitable, value-based care to local Medicare populations at neighborhood centers. Responsibilities include assessing and diagnosing patients, overseeing and administering primary care, prescribing medications, maintaining patient EMR data via Canopy and Greenway, collaborating with regional/central leadership to meet health quality goals, and working with Practice Managers to direct the center care team. New graduates are encouraged to apply; a Center Medical Director role may be considered for highly qualified candidates. The role emphasizes prevention, outcomes, reduced hospital admissions, and delivering an exceptional patient experience in outpatient primary care, within a value-based healthcare model.
