Job Summary
Provide outpatient primary care to adult patients in a value-based setting while managing chronic conditions, preventive care, and post-hospital follow-ups. Lead and collaborate with an interdisciplinary team including APPs, nurses, behavioral health, and social work to develop care plans for medically complex patients. Participate in population health huddles, quality improvement initiatives, and transitional care planning to reduce avoidable ER visits and hospitalizations. Serve as a clinical leader within the local market, mentoring providers and staff while using data and EHR tools to improve outcomes. Full-time outpatient schedule with smaller panels of 10-12 patients daily.
Required Qualifications
- MD or DO degree
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active or eligible for an unrestricted Ohio medical license
- No visa sponsorship available for the Columbus role
Desired Qualifications
- Primary care, outpatient medicine, community health, FQHC, or value-based care experience
- Clinical leadership experience
- Experience mentoring physicians, APPs, or care teams
- Comfortable caring for medically complex and underserved patient populations
- Strong communication, leadership, organization, and problem-solving skills
- Ability to work in a team-based, outcomes-driven care model
