Job Summary
Provide comprehensive outpatient primary care services, including evaluation, diagnosis, and treatment for a diverse patient population. Perform routine physical exams, preventive screenings, and health assessments while managing acute and chronic conditions such as diabetes, hypertension, asthma, and infections. Order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans. Educate patients on disease prevention, wellness, and lifestyle modifications, and collaborate with specialists and multidisciplinary teams for coordinated care. Maintain accurate documentation within the EMR system and participate in quality improvement initiatives. Required Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine
- Active Connecticut medical license (or eligibility)
- Strong clinical judgment, communication, and patient care skills
