Primary Care Physician at Oak Street Health delivering value-based, patient-centered care to Medicare populations in an in-clinic setting. Responsibilities include assessing and diagnosing patients at local clinics, directing and administering primary care, prescribing medications, maintaining EMR data, collaborating with leadership to meet health quality goals, and directing the center care team.
