Primary Care Physician

Primary Care Physician at Oak Street Health responsible for providing equitable, value-based care to local Medicare populations. Operates at a neighborhood primary care center with a dedicated care team to optimize patient outcomes, emphasize preventive medicine, and deliver an exceptional patient experience. Responsibilities include assessing and diagnosing patients, directing and administering primary care, prescribing medications, maintaining EMR data, collaborating with regional/central leadership to meet quality goals, and working with Practice Managers to guide the care team. New graduates are encouraged to apply; candidates may be considered for a Center Medical Director position. Knowledge of outpatient primary care, value-based care models, and collaboration within multidisciplinary teams is preferred.
