Job Summary
Primary Care Physician at Oak Street Health delivering equitable value-based care to local Medicare populations in a neighborhood primary care setting. Responsibilities include assessing and diagnosing patients, directing and administering primary care, prescribing treatments, maintaining EMR data, collaborating with leadership to meet health quality goals, and directing the center care team alongside Practice Managers. Qualified candidates hold an M.D. or D.O. with board certification (or eligible), possess an unrestricted state license and DEA license, and have US work authorization. Preferred qualifications include geriatrics fellowship, experience in geriatric outpatient care and value-based models, and bilingual proficiency. Anticipated hours 40 per week; salary range $174,070.00 - $374,920.00 depending on experience and geography.
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
