Primary Care Physician

Provide equitable, value-based primary care to Medicare populations at Oak Street Health clinics. Assess, diagnose, and manage patients; oversee and administer primary care; prescribe medications; maintain EMR data (Canopy and Greenway); collaborate with regional/central leadership to meet health quality goals; direct the center care team with Practice Managers; position may include Center Medical Director consideration; new graduates are encouraged to apply. This role emphasizes geriatrics, outpatient primary care, and delivering preventive, outcome-driven care within a value-based model.
