Primary Care Physician

Primary Care Physician at Oak Street Health provides value-based, equitable care for local Medicare populations. Responsibilities include evaluating and diagnosing patients, overseeing and administering primary care, prescribing medications, maintaining EMR data via Canopy and Greenway, collaborating with leadership to meet health quality goals, and directing the center care team with Practice Managers.
