Job Summary
Provide equitable value-based primary care to local Medicare populations at Oak Street Health's neighborhood centers. Assess and diagnose patients, oversee and administer primary care, prescribe and manage medications, maintain Patient EMR data via canopy and greenway, collaborate with regional/central leadership to meet health quality goals, and work with Practice Managers to direct the center care team. New graduates are encouraged to apply. Preferred qualifications include geriatrics fellowship, experience with geriatrics populations, experience in value-based care models, outpatient primary care experience, and bilingual proficiency where applicable. This full-time physician role emphasizes delivering preventive care and high-quality patient experience in a value-based healthcare setting.
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
