Primary Care Physician

Primary Care Physician at Oak Street Health providing value-based, equitable care to Medicare patients at a neighborhood primary care center. Responsibilities include assessing and diagnosing patients, overseeing and directing the primary care team, prescribing treatments, maintaining patient EMR data (Canopy and Greenway), and collaborating with regional leadership to meet health quality goals. Role supports new graduates; highly qualified candidates may be considered for Center Medical Director. Requires MD/DO with board certification (or eligibility) in Internal Medicine or Family Medicine, active State license, and active DEA license; US work authorization. Knowledge of outpatient primary care, value-based care, and ability to work with care teams to improve patient outcomes is preferred; bilingualism optional. The center is located in Zionsville, IN.
