Primary Care Physician

Primary Care Physician at Oak Street Health delivering value-based, equitable care to Medicare populations at neighborhood primary care centers. Responsibilities include assessing and diagnosing patients at local clinics, overseeing and directing primary care, prescribing and administering medications, maintaining Patient Electronic Medical Record data via canopy and greenway, collaborating with regional/central leadership to meet health quality goals, and working with Practice Managers to direct and manage the center care team, plus other duties as assigned. New graduates are encouraged to apply; highly qualified candidates may be considered for Center Medical Director. Preferred qualifications include Geriatrics fellowship, geriatrics experience, value-based healthcare experience, outpatient primary care experience, and bilingual proficiency where applicable. Supports US work authorization and medical licensure requirements; board certification in Internal Medicine or Family Medicine (or eligibility) and active DEA license are required.
