Primary Care Physician

Primary Care Physician to join a mission-driven health system in a fully in-person role at the Pittsburgh medical center. Provide comprehensive primary care to a panel of ~250 patients, seeing ~10 per day, within a multidisciplinary team including case management, behavioral health, pharmacists, and nutritionists. Lead transitional care plans and coordinate discharges from hospitals, ERs, and skilled nursing facilities. Accept same-day/next-day urgent appointments to prevent hospitalizations. Utilize eClinical EMR for documentation and coordination. Educate patients on chronic disease management and advanced directives. Open to occasional home visits. Board-certified in Internal Medicine or Family Medicine; licensed in Ohio; BLS certified. 15+ years hospitalist experience preferred.
