Primary Care Physician

Primary Care Physician sought to join a mission-driven healthcare provider in Cincinnati for a full-time, in-person role. Board-Certified Internal Medicine or Family Medicine physician with an Ohio license and BLS certification will provide comprehensive primary care to a panel of ~250 patients (approximately 10/day), lead a multidisciplinary team, develop transitional care plans, coordinate discharges from hospitals/ERs/SNFs, accept same-day/next-day urgent appointments, and use eClinical EMR for documentation and care coordination. Responsibilities include educating patients on chronic disease management and advanced directives, and supporting care for high-utilization, underserved populations, with open to occasional home visits. The position emphasizes high-touch, value-based care, collaborating with behavioral health, case management, pharmacists, and nutritionists, in a fully in-person setting with no remote work.
