Primary Care Physician (MD/DO)

Provide comprehensive primary care to older adult patients with full accountability for clinical outcomes and patient experience. Serve as a health coach guiding lifestyle improvements, coordinate specialty and hospital care, and lead an interdisciplinary team including Advanced Practice Providers. Partner with medical and operational leadership to meet quality and cost goals while managing a panel capped at approximately 450 patients. Maintain an appointment utilization of 18–21 slots per day and focus on prevention, care coordination, and behavior change within a value-based Medicare Advantage model.
