Job Summary
Primary Care Physician for ISNP program in institutional settings; provide comprehensive primary care to complex senior populations; coordinate care with an interdisciplinary team to support transitions of care, adherence to CMS/ISNP guidelines, and documentation in an EMR. Role emphasizes preventive care, chronic disease management, and alignment with value-based care initiatives within SNF/LTC and other institutional settings, with travel between facilities as needed. Requires MD/DO, board certification/eligibility in Family or Internal Medicine, and active license; BLS; DEA; TB testing within 12 months; Hep B vaccination; and participation in quality and risk-adjustment programs.
Required Qualifications
- MD or DO from an accredited medical school
- Completion of an accredited residency program in Family Medicine, Internal Medicine, or Geriatrics (preferred)
- Current, unrestricted medical license in the state of practice (or ability to obtain)
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active DEA license
- BLS certification
- Completion of all required health screenings (TB test within last 12 months)
- Hep B Vaccination (all 3 doses, titer or signed declination)
