Primary Care Physician in Adrian, MI delivering high-quality, cost-effective patient care in post-acute care settings. Collaborate with interdisciplinary teams, follow CMS guidelines, and coordinate with hospice, palliative care, and home care agencies for safe patient transitions. Required Qualifications
