Job Summary
Outpatient-only primary care physician role in a value-based care model serving older adults. Manage a patient panel with ~100 new patients ramping up, see ~18 patients per day, and enjoy a Monday–Friday schedule with one remote/work-from-home day per week. Work with a robust care team (EMTs, APPs, medical assistants, care managers) and dedicated acute care support to maximize patient interactions and minimize administrative burden. Competitive base salary with bonus potential and comprehensive benefits including medical/dental/vision, 401(k) with match, PTO, holidays, CME allowance, and malpractice coverage. Ideal candidate is Board Certified or Board Eligible in Internal Medicine, Family Medicine or Geriatrics, passionate about caring for older adults in a team-based, patient-centered environment with predictable hours and strong compensation.
