Job Summary
Provide comprehensive primary care services across multiple locations in Southwestern Pennsylvania, including Full Health, Hermitage Community Medicine, Horizon Family Healthcare, Horizon Family Medicine, and Partners in Health. Deliver patient-centered care in an employed model within a nonprofit health system, adhering to a day schedule from Monday through Friday. Maintain unrestricted Pennsylvania medical licensure and DEA certification while participating in the Value Incentive Plan and panel incentives. Join a network of over 800 outpatient sites serving diverse communities, contributing to nearly $2 billion in annual community benefits.
Required Qualifications
- Medical Degree (MD/DO)
- Accredited Family Medicine Residency Program Completion
- Board-certified in Family Medicine (or board-eligible for residency new graduates)
- Unrestricted Pennsylvania Medical License (or ability to obtain)
- DEA Certification
- A commitment to patient-centered care and excellent interpersonal skills
