Conduct primary care for a panel of 500–600 patients in Aldine, TX, with longer 60-minute new patient visits and 30-minute established follow-ups. Manage chronic disease in seniors within a team of three physicians, three NPs, and dedicated support staff including an MA, scribe, and RN for high-risk cases. Leverage in-house resources such as physical therapy, social work, nutrition services, and pharmacy support to deliver value-based outcomes. Maintain light shared call rotation with nurse triage, pooled across Texas providers for approximately two weeks annually. Join a growing employed group actively adding new patients monthly, with a steady ramp to a full panel. Spanish-speaking ability is strongly preferred given the predominantly Spanish-speaking patient population.
