Job Summary
Diagnose and treat acute and chronic conditions, conduct preventive screenings, and build long-term treatment plans for outpatient patients across the lifespan or adult care. Document in the EMR, coordinate with a multidisciplinary care team, and participate in evidence-based quality improvement work. Manage a shared, phone-only call rotation to protect evening and weekend time. Required Qualifications
- MD or DO
- board certified or board eligible in Family Medicine or Internal Medicine
- Active New York State medical license, or eligibility to obtain one
- DEA certification, or ability to obtain one
- Strong outpatient clinical skills
- commitment to community-based, patient-centered care
- Comfortable working within a physician-led, collaborative practice model
