Job Summary
Provide comprehensive primary care services for adult patients with a focus on prevention, chronic disease management, and wellness. Accurately document patient encounters to support value-based care initiatives and collaborate with an interdisciplinary team for coordinated, high-quality care. Participate in population health initiatives to identify opportunities that improve patient outcomes while building long-term relationships through compassionate, evidence-based practice. Effectively identify risk-adjustment diagnoses and help eligible patients enroll in advanced care management programs. Progress toward an average of approximately two patient visits per hour while maintaining a high standard of care.
Required Qualifications
- Board Certification in Family Medicine or Internal Medicine
- Significant clinical experience in a primary care setting
- Strong electronic medical record (EMR) documentation skills
