Job Summary
Provide elderly-focused primary care within a value-based care model to geriatric patients, delivering comprehensive, patient-centered care with an emphasis on chronic disease management, preventive care, and coordination across the healthcare team. See an average panel of ~500 patients, with visits typically 30–45 minutes and scheduling from 9:00 a.m. to 3:30 p.m., supported by dedicated coders and medical assistants to streamline documentation and care coordination. Utilize EMR systems (Epic or eCW) and data analytics to monitor outcomes, close care gaps, and drive quality metrics while fostering an exceptional patient and family experience. Lead care plan discussions, collaborate with specialists, and focus on improving patient engagement and value-based outcomes. Office hours are Monday–Friday, 8 a.m.–5 p.m., with compensation including base salary plus performance bonuses and potential clinic bonuses; benefits include PTO, health/dental/vision, and 401K with employer match.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a Florida State Medical License or already have a current, active Florida State Medical License
- Board certification or board eligible in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
- Experience: 2-6 years in a primary care setting, preferably managed care, or recent residency graduate
- Strong communication and interpersonal skills
