Primary Care Physician at Oak Street Health in York, PA: provide equitable and effective value-based primary care to local Medicare populations in a neighborhood center. Responsibilities include assessing and diagnosing patients, overseeing and directing the center’s primary care, prescribing medications, maintaining EMR data (Canopy and Greenway), collaborating with regional/central leadership to meet quality goals, and working with Practice Managers to direct the care team.
