Primary Care Physician

Provide equitable and effective value-based primary care to local Medicare patient populations at Oak Street Health centers. Responsibilities include assessing and diagnosing patients, directing and administering primary care, prescribing medications, maintaining Patient Electronic Medical Record data via canopy and greenway, collaborating with regional/central leadership to meet health quality goals, and directing the center care team with Practice Managers.
