Primary Care Physician at Oak Street Health to provide equitable, value-based primary care to local Medicare populations in a neighborhood clinic. Responsibilities include assessing and diagnosing patients, overseeing and directing the center care team, prescribing and administering medications, maintaining Patient EMR data via canopy and greenway, collaborating with regional/central leadership to meet health quality goals, and directing practice managers to manage the center care team.
