Primary Care Physician

Primary Care Physician at Oak Street Health providing value-based, equitable care to Medicare patients at a local clinic. Role involves assessing and diagnosing patients, overseeing and administering primary care, prescribing medications, maintaining Patient EMR data, and collaborating with regional/central leadership to meet health quality goals. Works with Practice Managers to direct and manage the center care team. New graduates encouraged; potential consideration for Center Medical Director. Qualifications include MD or DO with board certification (or eligibility), active state license, active DEA license, and US work authorization. Preferred include geriatrics fellowship and experience in geriatrics, value-based models, outpatient primary care, and bilingual proficiency where applicable.
