Primary Care Physician

Primary Care Physician needed at Oak Street Health to provide value-based, quality-focused outpatient care for Medicare populations at a neighborhood primary care center. Responsibilities include assessing and diagnosing patients, directing and administering primary care, prescribing medications, maintaining EMR data, collaborating with leadership to meet health quality goals, and directing the center care team with Practice Managers. New graduates are encouraged to apply; advanced roles like Center Medical Director may be considered. Requires MD/DO, board certification or eligibility in Internal Medicine or Family Medicine, active state licensure, active DEA license, and US work authorization. Knowledge of value-based care, experience in outpatient primary care, and ability to operate within a team-based, outcomes-driven model are preferred.
