Primary Care Physician

Oak Street Health seeks a Primary Care Physician to provide equitable, value-based care to local Medicare populations at our neighborhood primary care centers. Responsibilities include assessing and diagnosing patients, overseeing and administering primary care, prescribing and managing medications, maintaining patient EMR data via canopy and Greenway, collaborating with regional and central leadership to meet health quality goals, and working with Practice Managers to direct and manage the center care team.
