Primary Care Physician

Provide equitable, effective value-based primary care to local Medicare patient populations at Oak Street Health’s neighborhood centers. Assess and diagnose patients, oversee and direct the center care team, prescribe treatments, maintain EMR data (Canopy and Greenway), and collaborate with leadership to meet health quality goals while delivering preventive and patient-centered care. New graduates are encouraged to apply and experienced clinicians may be considered for Center Medical Director. Focus on geriatrics and outpatient primary care within a value-based care model with competitive bonuses.
