Job Summary
Primary Care Physician at Oak Street Health providing equitable value-based care to Medicare populations. Responsibilities include assessing and diagnosing patients, overseeing and directing primary care, prescribing medications, maintaining patient EMR data via Canopy and Greenway, collaborating with regional/central leadership to meet health quality goals, and working with Practice Managers to direct the center care team. New graduates are encouraged to apply; potential for Center Medical Director role. Requires MD/DO with board certification or eligibility, active state license, active DEA, and US work authorization. Center-focused, outpatient primary care in a value-based healthcare model.
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
