Primary Care Physician

Primary Care Physician needed at Oak Street Health to provide equitable, value-based care for Medicare patients at a neighborhood primary care center. Responsibilities include assessing and diagnosing patients, overseeing and directing primary care, prescribing medications, maintaining EMR data (Canopy and Greenway), collaborating with regional/central leadership to meet health quality goals, and working with Practice Managers to lead the center care team. New graduates are encouraged to apply. Preferred qualifications include geriatrics fellowship, experience with geriatric populations, experience in value-based healthcare, and outpatient primary care experience. Center roles may also include Center Medical Director positions. Location: Montpelier, VT.
