Job Summary
Primary Care Physician at Oak Street Health providing equitable, value-based care to Medicare patients in a neighborhood primary care network. Responsibilities include assessing and diagnosing patients, directing and coordinating the center care team, prescribing medications, and maintaining patient EMR data via Canopy and Greenway. Collaborates with regional/central leadership to meet health quality goals and works with Practice Managers to manage the center. New graduates encouraged to apply; potential for Center Medical Director role. Relocation package available case-by-case. Anticipated weekly hours: 40; full-time role with competitive salary and bonuses.
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
