Job Summary
Provide equitable, value-based primary care to Medicare patients at Oak Street Health clinics. The role involves assessing and diagnosing patients, overseeing the primary care team, prescribing and administering treatments, maintaining EMR data via Canopy and Greenway, collaborating with regional/central leadership to meet health quality goals, and directing the center care team with Practice Managers. Emphasis on caring for older adults, outpatient primary care production in a collaborative, team-based setting with focus on preventive medicine and delivering an unmatched patient experience. New graduates are encouraged to apply; there is potential consideration for a Center Medical Director role. Knowledge of geriatrics, value-based care, outpatient systems, and bilingual proficiency where applicable are preferred.
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
Desired Qualifications
- Fellowship training in Geriatrics
- Experience practicing medicine among geriatric populations
- Experience operating in a Value-Based Healthcare Model
- Experience working in a collaborative setting to ensure positive health outcomes
- Experience in outpatient primary care settings
- Bilingual proficiency where applicable
