Provide equitable value-based primary care to local Medicare patient populations at Oak Street Health’s network of neighborhood primary care centers. Operate within a single medical center supported by large care teams to emphasize quality of care and preventive medicine, with bonuses tied to outcomes and patient experience. New graduates are encouraged to apply; high qualifications may qualify for Center Medical Director. Responsibilities include assessing and diagnosing patients, directing primary care, prescribing treatments, maintaining EMR data via Canopy and Greenway, collaborating with leadership to meet health quality goals, and coordinating with Practice Managers to lead the center care team.
