Job Summary
Deliver comprehensive outpatient primary care to a small panel of ~100 medically and socially complex patients, primarily in patients' homes and community settings in the Greater Boston area. Serve as the PCP for those in the Compass PLUS program, consulting with other providers caring for high-risk patients. Work with multidisciplinary teams in homes and community spaces, developing personalized care plans addressing medical, behavioral, and social needs. Collaborate with patients and teams to improve health outcomes and reduce unnecessary hospital use; support transitions between inpatient, emergency, and outpatient care with the goal of reintegration into traditional outpatient care. Enjoy significant clinical autonomy within a collaborative environment; panel size enables focused, intensive care with no evening or weekend call obligations. Local travel required with occasional regional trips (1-2 times per month). Qualifications include MD/DO in Internal Medicine or Family Medicine, board certification or eligibility, MA license and DEA, outpatient experience in high-need environments, and interest in population health and health equity.
