Primary Care Physician- Compass PLUS Program

Primary Care Physician for the Compass PLUS program, delivering comprehensive outpatient care in homes and community settings to a small panel (~100 patients). Responsibilities include managing coordination with multidisciplinary teams, creating personalized care plans addressing medical, behavioral, and social needs, consulting with other PCPs, supporting transitions between inpatient, emergency, and outpatient care, and collaborating with patients and teams to reduce unnecessary hospital utilization. Work is centered in the Greater Boston area with local travel; no evening or weekend call obligations. Qualifications include MD or DO in Internal Medicine or Family Medicine, board certification or eligibility, experience in outpatient/high-need environments preferred, interest in population health and health equity, ability to work in collaborative care, and Massachusetts license and DEA eligibility.
