Job Summary
Provide comprehensive assessment, diagnosis, treatment, and care coordination for older adult patients with acute and chronic illnesses, maintaining accountability for outcomes across panels of up to 450 individuals. Lead a care team including medical assistants and coordinators while engaging with hospitalists, transitional care teams, and specialists for out-of-office patients. Manage center operations by covering for absent providers and recruiting patients and additional staff as needed. Adhere to strict quality metrics, regulatory compliance, and performance standards while championing a culture of accountability and passion. Work independently and collaboratively with managers and medical directors to ensure guideline compliance and risk management.
Required Qualifications
- Primary Care Physician
- Active role in the management of their center
- Active role as needed in recruiting patients for the center
- Active role as needed in recruiting additional providers for the company
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook
- Ability and willingness to travel locally as needed in their market
- Ability and willingness to travel nationally for initial training (2 weeks)
- Ability and willingness to travel occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
- Ability to function independently
- Ability to collaborate with other health care professionals
- Willingness to continuously work to develop and improve leadership skills
- Willingness to work with the care teams towards achieving high quality outcomes
- Willingness to continuously and systematically improve their practice of medicine
