Job Summary
Provide primary care for older adults with acute and chronic illnesses, managing up to 450 patients while coordinating care for those in hospitals, SNFs, and homes. Lead a care team including medical assistants and coordinators, engage with hospitalists, and collaborate with managers on risk and quality management programs. Act as a health coach to help patients set goals, simplify treatment plans, and adhere to departmental metrics such as admissions, appointment slots, and CGCAHPS scores. Maintain strict compliance with regulatory standards, policies, and procedures while championing a positive culture of accountability. Work independently and with advanced practice practitioners, utilizing independent judgment for complex cases.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
- Fluency in English, verbal and written
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
- Flexible to work evening, weekends and/or holidays as needed
