Job Summary
Primary Care Physician (PCP) with ChenMed leading patient-centered, outcomes-focused care for an older adult panel. Provides independent care for acute and chronic illnesses, leads a care team, collaborates with hospitalists and ancillary staff, and engages in risk and quality management programs to improve patient health and stay aligned with center goals. Demonstrates accountability for patient outcomes, coaches patients toward health goals, and participates in recruiting patients and providers as part of center leadership. Requires MD or DO in Internal Medicine, Family Medicine, or Geriatrics; active state medical license; DEA number; BLS; and board certification/eligibility in relevant specialty. Total patient panel up to 450; travel for training and occasional national activities may be required. Fluency in English; willingness to work flexible hours including evenings/weekends/holidays; ability to travel locally and regionally as needed.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
